Maybe you can put this info: "Most questions can be answered on a scale from 1 to 5 going from “strongly disagree” to “strongly agree”. After each question there is space for your comments, and there are also some open questions." on every page, at the side. Because after the first question I forgot if 1 or 5 was the best...
