Besides, how do you enforce one questionaire/pp if you want guarantee anonimity? A nice solution would be with a random token system, but I guess that would be overkill ;-).
Or make them log in but guarentee they can do it anonynously ;) As you could print multiple questionnaires too, I figured people would be too lazy to fill in the questionnaire too many times (and besides, I do log their IP and when they fill it in ;) I can track abuse) -Kenny
