Questionnaire is not on the https server, that's a security risk :), I don't want my anyone sniffing out my responses Besides, how do you enforce one questionaire/pp if you want guarantee anonimity? Maybe you can put this info: "Most questions can be answered on a scale from 1 to 5 going from “strongly disagree” to “strongly agree”. After each question there is space for your comments, and there are also some open questions." on every page, at the side. Because after the first question I forgot if 1 or 5 was the best... No, new entries are added everytime. Only a check made for people who go back and forth a through their history.
